Your First Visit

Our friendly team will make you feel at home from the moment you arrive. You’ll be asked to complete simple paperwork, including medical history and patient information forms. For convenience, new patient forms are also available online to fill out before your visit.

During your first appointment, you’ll meet Dr. Amy, Dr. Carey, Dr. Chris, or one of our Associates, who will take the time to listen to your needs and discuss treatment options. A typical first visit lasts about an hour.
